The forms will need to be filled out, printed and from there you can either fax them to us (978 274 5672), email them to firstname.lastname@example.org or mail them to us at:

Remember that it is a $200 per week deposit (e.g. if you are signing up your child for four weeks of camp, then a check for $800 should accompany your forms). If you are registering before December 1st or after April 1st, then the full amount is due upon registering.

** Forms have a deadline of May 15th, however if you register after this date, the forms need to filled out and submitted as soon as possible!
